HIPERTENSÃO SECUNDÁRIA - RENOVASCULAR:
DIAGNÓSTICO E TRATAMENTO

O presente artigo revisa o importante papel do diagnóstico e
tratamento da doença renovascular, que pode ou não ser causa
de hipertensão arterial. Infelizmente ainda não dispomos
na atualidade de evidências científicas robustas que nos
orientem sobre o melhor momento e a melhor forma terapêutica para
pacientes portadores de doença renovascular. É certo que,
independentemente da opção terapêutica utilizada,
além da necessidade de se alcançar melhor controle da pressão
arterial e preservação da função renal, é
imperativo que todas as co-morbidades presentes sejam corrigidas, quando
se buscam melhores resultados finais pós-intervenção.
Assim, a investigação precoce baseada em critérios
clínicos de probabilidade somada ao auxílio de exames complementares
permitirão maiores chances de acerto diagnóstico, cuja certeza
só existirá após a realização da intervenção
escolhida. Infelizmente a mesma certeza que temos nas melhores opções
diagnósticas não dispomos em relação aos resultados
tardios no que tange à evolução clínica dessa
população. Necessário será aguardarmos os
resultados de estudos em andamento e que estes possam revelar nossas maiores
dúvidas quanto à melhor forma de tratamento.

SECONDARY HYPERTENSION - RENOVASCULAR HYPERTENSION:
DIAGNOSIS AND TREATMENT
The present article revises the diagnosis and treatment of renovascular
disease that might cause hypertension. There are no evidences in order
to identify which patients will benefit with treatment and what is the
best approach for patients with renovascular disease. The association
of clinical signs and screening tests allow increased likelihood of diagnosis.
However, the diagnosis of renovascular hypertension is confirmed after
the intervention. The interventional treatment of renal artery stenosis
intends to provide better blood pressure control and the preservation
of renal function and the risk factors have to be aggressively treated
after the intervention. The clinical outcomes of patients with renal artery
stenosis treated with interventional techniques are unknown. Future studies
addressing this question will clarify the best treatment for patients
with renal artery stenosis.
